The Center for Healthy Lifestyles is committed to improving the overall health of our community. That’s why we offer a number of regularly scheduled health screenings. Some of our most popular screenings include:

OSF Heart Card
The OSF Heart Card is a wallet-sized card containing a personal health history and a copy of the patient’s electrocardiogram (EKG). It is recommended that the heart card be carried in a wallet or purse at all times. In the event of a medical emergency, healthcare providers can reference the baseline EKG/heart card. Personal Wellness Profile
Results from a 75-question health and lifestyle survey and clinical measurements provide you with an individualized report including recommendations for improving nutrition, body composition, and reducing risks for disease.
